Our social services dept focuses on building trusted relationships with both patients & loved ones in order to provide the best transition back into the community or becoming part of the Sutton Home. Our services include:
- Adjustment Counselor
- Monthly care plan meetings for short term residents
- Comprehensive Discharge planning which includes home care services, equipment, etc.
